Position Overview:
The Early ACCESS Service Coordinator works in partnership with families by assisting them in obtaining the resources, services and assistance they need. The Service Coordinator is responsible for coordinating all services across agency lines and serving as the single point of contact for the child and family eligible for Early ACCESS and early intervention services.

Qualifications:
The Early ACCESS Service Coordinator must hold a minimum of a Bachelor of Arts degree and have excellent written and verbal communication. Requires licensure by the State of Iowa Board of Educational Examiners. Education or related work experience in early childhood education, social work, nursing or a related field is beneficial. Individual must be willing to complete Iowa certification requirements for the position. The individual must possess organizational skills and complete tasks in a timely manner. The candidate must have the experience or capacity to demonstrate positive interpersonal skills, cooperative teamwork, and multi-agency coordination.

Responsibilities:
1. Understands and applies the use of family-centered practices
2. Understands and abides by confidentiality standards.
3. Available on a year-round basis.
4. Completes Early ACCESS Service Coordinator training, and maintains-Knowledge of IDEA/Part C (Early ACCESS), the system of payments for services in Iowa and other pertinent information.Knowledge of community, state, and national resources and services.
5. Participates in Regional Early ACCESS Service Coordinator meetings.
6. Provides initial face-to-face contact with families and monthly follow-up based on family needs.
7. Coordinates services across agency lines and collaborates with all partners in order to meet the needs of the family.
8. Serves as the contact person, available year-round in helping families to obtain the services and assistance they need.
9. Coordinates and arranges necessary evaluations and assessments.
10. Informs families, and facilitates access to resources and services that could help the child and family.
11. Educates families on the Individualized Family Service Plan process.
12. Participates in writing, reviewing and updating the Individualized Family Service Plan.
13. Facilitates development of a transition plan prior to the child exiting Early ACCESS.
14. Provides timely, accurate documentation of Service Coordination activities as requested by Regional Director and Early ACCESS Regional Coordinator.
15. Assumes other duties and responsibilities as may be assigned by the Regional Director.

Salary Range: Per Year
Shift Type: Full-TimeEstimated Salary: $20 to $28 per hour based on qualifications.

The check will bounce and the victim is left responsible.
